Application for Tuition Payment Installment Plan and/or Financial Assistance
**APPLICATION IS DUE Friday, May 27, 2022 **
The tuition for each participant in Leadership Jacksonville is $4,500 and includes all expenses associated with two overnight retreats plus materials, facilities and meals for all program days. Tuition will be invoiced Friday, July 1, 2022 and full payment of tuition is due by Monday, August 1, 2022.
Tuition Payment Installment Plan
In the event that a participant is unable to pay the full tuition by Monday, August 1, 2022, alternative arrangements must be made and mutually agreed upon, with the final payment due no later than Monday, May 1, 2023.
If a payment plan is approved, Leadership Jacksonville will automatically charge your Visa/Mastercard account each month.
Financial Assistance
A limited number of partial scholarships are available to assist those who demonstrate financial need to cover their tuition expenses. The minimum contribution that an individual must make is $500. Since funds are limited, applicants are encouraged to seek funds from employers, boards or other sources.

The Executive Committee of the Leadership Jacksonville Board of Directors makes final decisions in scholarship awards.
The application form must be submitted by Friday, May 27, 2022, midnight.
