This page contains the information and forms necessary to participate in the Leadership Jacksonville flagship program. Please read all information in its entirety and respond as requested. We must receive ALL responses by midnight, Friday, May 24.

There are two steps required. Click on the adjacent boxes to complete.
- Read and sign the Expectations and Policies.
With your signature, you will be acknowledging that you have read, understand and will adhere to the expectations policies. You may download or print for future reference. - Complete and submit the Program Planning and Payment Form.
Important Dates:
- Meet the Class Reception at the Jessie Ball duPont Center: Tuesday, June 4, 2024
- Opening Dinner at UNF: Tuesday, July 30, 2024 (you will be given homework to complete prior to dinner)
- MANDATORY Opening Retreat at World Golf Village: Thursday and Friday, August 15 – 16, 2024
